You need to have a family doctor who refers you to the Pain Clinic
Our nurses go over the referral letters and assess the referrals
This is not a "first come, first serve" clinic. There are some pain syndromes for which we can change the future of the chronic pain and reduce the consequent disabilities by early intervention. They will be seen as soon as possible. A good example is Shingles and Complex Regional Pain Syndrome, CRPS.
You will be called to confirm your appointment. UNFORTUNATELY it may take over 12 months before we call you. We are working hard to reduce this time; however the demand for this service is far beyond the resources.
Register at admitting in the main lobby, bring your Ontario Health Card.
You will be seen in Ambulatory Care.
There is 2 main part for Pain Clinic, The 1st one is located in Am.Ca. and this is the one you need to come for the 1st time.
The 2nd visit could be arranged to review your medical management, to do procedure under Ultrasound or by the guidance of X-ray. In the last case scenario, you would be asked to come to Diagnostic-Imaging department.
Please bring any result of the radiological investigations, like CT scan or MRI that was done in another center. You family doctor may already has sent the results of your most recent study. If you do not have the results, it is helpful if you get a copy of the report. You could ask that the reports be faxed to us. All referrals have to be Faxed to our clinic at 519-436-2545.
If you want us to send a copy of our investigations and clinical notes to WSIB, please inform them that you are being seen in our clinic, they will contact us.
